ATTENTION: It is recommended you call your insurance company to verify you have in-network benefits with our practice, as well as find out if your insurance requires a referral in order to be seen by one of our providers. It is becoming more common for insurance companies to require a referral.
- It is the PATIENT’S responsibility to find out if their insurance requires a referral.
- If your insurance company DOES require a referral, it is YOUR responsibility to provide our office with the referral.
- If your insurance company denies payment due to a referral not being obtained, the PATIENT is responsible for full payment of any charges incurred during their visit.

Business Office Hours
Our Business Office is open Monday through Friday 8am – 4:30pm. Anyone coming to our office for medical records, forms, or other business office inquiries need to come between these hours.

No Surprises Act
As part of the patient transparency protections, Congress requires all providers, including providers furnishing care in the physician office setting, to provide a good faith estimate (GFE) for care to uninsured or self-pay patients. Additionally, the law requires providers and insurers to furnish an advanced explanation of benefits to insured patients.
If you believe you’ve been wrongly billed, you may submit a complaint at nsa-idr.cms.gov/consumercomplaints or call the No Surprises Help Desk at (800) 985-3059. No Show Policy
If you no-show for your appointment or cancel less than 1 business day in advance, including procedures, the no-show fee will be as follows:
Office Visit $50
Physical Therapy $50
MRI $100 *If the MRI is a double-study, you will be charged $200

Most insurance companies have numerous plans under their umbrella for which you are responsible in calling to make sure we are in-network. Due to the number of plans and constant changes by insurance companies, we are unable to stay current with our in/out of network status or our participation with each and every plan.
